INSURANCE
Crescent Park Physicians are out-of-network providers for all insurance carriers. You will be given a receipt for the services provided to submit to your insurance company with all necessary information.
Insurance that includes out-of-network benefits often reimburses 20-90% of fees.
In checking with your carrier, it may help to report the codes that will be billed.
You are responsible for understanding your benefits and whether or not you have out of network benefits, any deductible for out of network benefits you must meet,
and the dollar reimbursement amount for the following codes:
Initial Evaluation

Half Hour Medication Management Appointment
99214 or 99213 (depending on complexity) and 90833
Hourly Psychotherapy with or without Medication Management Appointments
99214 or 99213 (depending on complexity) and 90836
CANCELLATION POLICY
Crescent Park Physicians ask that patients provide 48 business hours notice prior to cancelling any appointment. If patients do not cancel in this time frame or fail to come to an appointment, they will be charged the full fee. This excludes any medical emergencies.
GOOD FAITH ESTIMATE NOTICE
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance, or who are not using insurance, an estimate of the expected charges for medical services, including psychotherapy and medication management services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy and and medication management services. You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. Make sure to save a copy or picture of your Good Faith Estimate.
